In Visual Studio 2019 all extensions previously located in the main menu are moved into the Extensions menu:

It adds unnecessary clicks when you actively work with these extensions. Luckily, Evgeny K released yesterday the Extensions in Main menu extension that allows choosing between the new and old behavior by bringing up all or selected extensions back into the main menu:
